1. Use the attached form or download from
the link below.

2. Print and completely fill it in.

3. Attach your application form with:

    a). Two pictures:
         1 of your face and 1 full body.
  b) A copy of your valid passport

4. Send your application form with the
     attachments to:

5. Transfer the US$ 300 application fee to:

Application Deadline: September 30, 2008

If you are accepted as a contestant, you will be notified by E-mail.

If you are not qualified for the contest, we will return your application fee minus 20US$ for bank transfer fee.

The Organizer (Tiffany's Show Pattaya) will provide full complimentary accommodation and meals to all the contestant at Woodland Hotel & Resort Pattaya.
